Pengumuman kepada Mahasiswa PPG Dalam Jabatan UM Palembang Tahun 2023 Angkatan II untuk melakukan Lapor Diri melalui formulir online berikut LaporDiriDALJAB20232 dari Tanggal 15 s.d 16 September 2023

Persiapkan File Scan Berkas-berkas di Bawah Ini, Sebelum Mengisi Formulir Lapor Diri:

  1. Format A1 dan Pakta Integritas (format diunduh dari SIMPKB)
  2. Biodata Mahasiswa (format diunduh dari SIMPKB)
  3. Surat Pernyataan Ijin Kepala Sekolah (format diunduh dari SIMPKB)
  4. Scan Ijazah S1
  5. Scan Transkrip Nilai S1
  6. Scan Kartu Identitas KTP/SIM
  7. Scan Pas Foto Berwarna dimensi 4 x 6 cm Harus foto terbaru (3 bulan terakhir), File Asli Studio (bukan foto cetak yang di scan, atau foto edit wajah)
  8. SKCK (dari Kepolisian setempat)
  9. Surat Keterangan Sehat (dari Puskesmas/RSUD setempat)
  10. Surat Bebas NAPZA (dari BNN/Kepolisian/Puskesmas/RSUD setempat)
  11. Scan NPWP (bagi yang memiliki)
  12. Scan Kartu Keluarga (Persyaratan LPTK)
